Who You Are

We are seeking an experienced Locum Adult Social Worker to support residents living in an Extra Care Housing scheme during a period of service change. As a compassionate and dedicated professional, you will possess the necessary qualifications and experience in adult social care, particularly with older adults and housing-based support services. You will be adept at working autonomously while managing complex caseloads in a dynamic environment.

What the Job Involves

The role involves undertaking Care Act-compliant assessments and reviews to ensure residents receive appropriate care during the transition from an Extra Care Housing model. Key responsibilities include:

  • Completing timely and proportionate Care Act needs assessments and reviews.
  • Supporting residents through transitions, ensuring updated care and support plans.
  • Collaborating with housing providers, care providers, commissioners, and health professionals.
  • Identifying and managing risks associated with changes to care provision and accommodation.
  • Conducting Mental Capacity Assessments and participating in Best Interest decision-making.
  • Ensuring individuals, families, and advocates are informed and supported during transitions.
  • Maintaining accurate case records in line with statutory and organisational requirements.

Skills

  • Qualified Social Worker with Social Work England registration
  • Experience in adult social care, ideally with older people and/or housing-based support services
  • Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding duties
  • Experience in assessments and reviews in Extra Care or supported housing settings
  • Confidence in managing complex caseloads in a change environment
  • Experience with service transitions and provider changes (desirable)
  • Partnership experience with housing providers (desirable)
  • Knowledge of commissioning and care provider handover processes (desirable)
