The Role
ECC Adult Social Care, through effective practice, continues in its
transformational shift from a focus on long-term care support, to a model
with four key principles:- prevention; early intervention; enablement and
safeguarding. This will enable people, their families and carers to be able
to access information and tools to live healthily and independently.
Providing an opportunity to respond pro-actively to growing service demand
in a sustainable way, within the financial constraints of the Council.
The Team Manager within our Discharge To Assess Team, leads a social care
team, holding them accountable for performance and delivering improvements.
Working with the Service Manager, contributing to the development of
strategy and ensuring a service which adapts responsively to change, in
particular in how adults are supported out of hospital and supported within
the community admission avoidance.
With proven ability to focus on results, develop innovative solutions and
build strong working relationships with partners, this leader will possess
a deep understanding of working in an extremely challenging financial
environment.
Your key responsibilities will include:
The Team
The ASC Discharge to Assess service, focus on assessing adults post
hospital discharge . However, they also work closely with health, who are
the lead agency in discharging adults, in providing advice and support on
discharge pathways to ensure effective transition out of hospital and to
prevent an readmission into hospital.Alongside this, they will work
directly with adults in the hospital in respect of safeguarding, complex
MCA’s or other complexities.
The ethos being to support Adults to leave hospital, when safe and
appropriate to do so, and continuing their care and assessment out of
hospital. Wherever possible Adults will be supported to return to their
home for assessment.
Support is focussed on enabling adults to remain as independent and
self-caring as possible in their own homes. However, some Adults may have
been discharged to a temporary care home environment to aid their recovery
and for a period of assessment to be able to identify their short- or
long-term care and support needs.
We are seeking an inventive, proactive leader who engages a strengths-based
approach to leadership in a multi-disciplinary arena that can at times be
challenging but rewarding as you see the team grow in confidence. You will
support and manage the team to work with adults, and their families, who
have a variety of strengths and complex social needs, to live as
independently as they can. You will have experience of making safeguarding
personal and positively identify and manage risk with the multi-agency
team. You will play an integral role in implementing Essex County Councils
early intervention model, promoting choice and control.
In addition, you will represent the team in a range of meetings both
internal and with our partner agencies. You will be working in
collaboration with health care professionals and care providers using a
multi – disciplinary approach to ensure the adults receive the right
support at the right time.

The team operates on a 7-day rota basis. However, you will be required to
work 37 hrs per week - Monday to Friday 9 to 5:30.

relationships with partner agencies and the local community to provide a
supportive framework for vulnerable adults.

licence and car - or the ability to meet the mobility requirements of
the role through other means - is essential.
